The platform was designed in collaboration with patients and clinicians to meet the needs of both. It is easy to use and built on validated Patient Reported Outcomes Measures (PROM) questionnaires.

How it works
Patients complete a regular PROM using their mobile phone. Clinicians access results in a secure dashboard.
Send monitoring form
Patients or their carers are sent a link to their online questionnaire by SMS.
Review the data
A care coordinator is alerted to significant changes in the patient’s symptoms.
Escalate and give treatment
Care coordinators can arrange urgent appointments or inform clinicians where results need further assessment.
Support self care
Coordinators send patient guidance on self-management to help the patient manage symptoms without having to come into the clinic.
Our work with Guys and St. Thomas Hospital
We worked with the rheumatology team at Guys and St. Thomas’ Hospital to set up a Remote Monitoring service for long-term patients in their care. Following it’s launch early findings show:
87.6%
of patients who were invited to participate in remote monitoring engaged with the service at Guy’s and St Thomas’ NHS Foundation Trust.
77.5%
of patients with rheumatoid arthritis had their flare-up identified early.
